§ 14-74. Applications for certificate of competency.

  • Applications for a certificate of competency shall be submitted to the city in writing on forms prescribed by the city, which shall require the following information:

    (1)

    Certified contractor. An application for a new or renewal certified contractor certificate of competency shall contain:

    a.

    The name, address and business telephone number of the applicant;

    b.

    The state contractor certificate number of the applicant with a photocopy of such certificate attached to the application;

    c.

    The notarized original signature of the applicant; and

    d.

    A statement that the sole purpose of the certificate of competency is to facilitate the issuance of building permits while assuring compliance with F.S. § 489.131(5).

    (2)

    Registered contractor.

    a.

    The city shall not issue a new certificate of competency for any registered contractor category, and shall not accept any application for a new registered contractor certificate of competency after the effective date of the ordinance form which this division is derived.

    b.

    The city shall renew a certificate of competency for a registered contractor holding a valid and current certificate of competency issued prior to the effective date of the ordinance from which this division is derived, until the category is repealed or the certificate of competency is invalidated as provided in this division. An application for a renewal registered contractor certificate of competency shall contain:

    1.

    The name, address and business telephone number of the applicant;

    2.

    The certificate of competency number issued to the applicant for the preceding year with a photocopy of the application form for such preceding year attached to the application; and

    3.

    The notarized original signature of the applicant.

    c.

    After the effective date of the ordinance from which this division is derived, a registered contractor holding an expired certificate of competency shall have one opportunity to activate such certificate as follows:

    1.

    No later than 90 days after the effective date of the ordinance from which this division derives, the city shall provide written notice to all registered contractors on file with the city, whose certificates of competency expired on or before such effective date, that such contractors must apply for renewal within 30 days after receipt of the notice. The notice shall be mailed by certified mail, return receipt requested, to the last known address of the contractor on file with the city.

    2.

    A registered contractor may appeal for an extension to the 30-day time limit by filing written notice with the city council. Such appeal shall comply with the procedures established in the land development code.

    3.

    The applicant shall pay the fee for activation of the registered contractor certificate of competency.

    4.

    Once activated, the registered contractor certificate of competency may be renewed as provided in this section.

    (3)

    Local specialty contractor.

    a.

    An application for a new local specialty contractor certificate of competency shall contain:

    1.

    The name, address and business telephone number of the applicant;

    2.

    The title of the specialty contracting category;

    3.

    The notarized original signature of the applicant;

    4.

    A statement whether the applicant is an individual, sole proprietor, or qualifying agent for a partnership, corporation, joint venture, or other business entity, and the name of the business entity;

    5.

    The names and addresses of all partners of the partnership, officers and directors of the corporation, joint ventures of the joint venture, or owners or individuals associated with the business entity; and

    6.

    Any other information required by the board to prove compliance with the requirements of this division.

    b.

    An application for a renewal local specialty contractor certificate of competency shall contain:

    1.

    All information required for a new certificate of competency; or

    2.

    The original application for a certificate of competency and a notarized statement attesting to the accuracy and validity of the information contained on such original application.

    (4)

    Journeyman.

    a.

    An application for a new journeyman certificate of competency shall contain:

    1.

    The name, address and business telephone number of the applicant;

    2.

    The title of the journeyman category;

    3.

    The notarized original signature of the applicant; and

    4.

    Any other information required by the board to prove compliance with the requirements of this division.

    b.

    An application for a renewal journeyman certificate of competency shall contain:

    1.

    All information required for a new certificate of competency; or

    2.

    The original application for a certificate of competency and a notarized statement attesting to the accuracy and validity of the information contained on such original application.
