§ 18-173. Same—Application.

  • A written application for a permit for solicitation by a charitable organization required by this division shall be sworn to and filed with the city clerk; the application shall contain the following information:

    (1)

    Name of the charitable organization applying for a permit to solicit and the address of its headquarters.

    (2)

    Names and address of its principal officers and management.

    (3)

    The purpose for which any receipts derived from such solicitations are to be used.

    (4)

    The name of the person by whom the receipts of such solicitation shall be disbursed.

    (5)

    The name, address and telephone number of the person who will be in direct charge of conducting the solicitation.

    (6)

    An outline of the method to be used in conducting the solicitation.

    (7)

    The time when such solicitation shall be made, giving the proposed dates for the beginning and ending of such solicitations.

    (8)

    The amount of any wages, fees, commissions and expenses, or emoluments to be expended or paid to anyone in connection with such solicitation, together with the manner in which such wages, fees, expenses, commissions or emoluments are to be expended, to whom paid an amount thereof.

    (9)

    Financial statement for the last preceding fiscal year of any funds collected by the activities set out in section 18-172 by the organization or persons seeking a permit for such solicitation, such statement giving the amount of money so raised, together with the cost of raising it, and the final distribution thereof.

    (10)

    A full statement of the character and extent of the charitable, religious, educational or philanthropic work being done by the applicant organization within the city.

    (11)

    A copy of the organization's certificate from the secretary of state, pursuant to F.S. ch. 496, known as the Solicitation of Charitable Funds Act.

    (12)

    A copy of its determination of tax exempt status by the Internal Revenue Service of the United States of America.
