§ 58-33. Application procedure.

(a)

Application for a right-of-way construction permit shall be made on a form provided by the city and shall include the following information:

(1)

Name, address, and telephone number of the applicant, and contact person if the applicant is a corporation, partnership or other legal entity.

(2)

Description of the work to be performed, including location, estimated value of construction, estimated dates of commencement and completion, and purpose of the construction.

(3)

Name, address, and telephone number of the contractor performing the work, if different from the applicant or the applicant's contact person.

(b)

The city manager or his designee may require plans, drawings, surveys or specifications to be submitted with any application, based on the nature and complexity of the work proposed.
